Clinical Consulting ServicesOur Consultant Pharmacy Division is flexible enough to integrate the facility's policies and procedures. The primary responsibility of our Consultant Pharmacist is to provide the facility with a complete, efficient, and safe drug delivery system. Our consultants work closely with the prescribing physicians to ensure the medical appropriateness of the drug therapy, as well as monitoring for proper drug and allergy interactions. The monthly fee includes all hours needed to complete services being provided. If special projects or developments require more time than specified, the facility's administration must advise Stat-Care for prior approval. Our consultants manage 30% fewer beds than the average long term care consultant in Massachusetts allowing additional time per resident. The pharmacy consultant will complete one med pass per month to ensure accurate medication administration. The Consultant Pharmacist possesses the appropriate educational credentials
as required by the Massachusetts Board of Registration in Pharmacy and is a
member in good standing of the American Society of Consultant Pharmacists.
The Consultant Pharmacist will take whatever steps are appropriate to assist
the facility in conforming to all state and federal regulations. All consultants
have laptops and printers to provide immediate documented recommendations.
Monthly Pharmacy ReviewsThe consultant pharmacist will review the clinical chart for each resident every 30 days or more often as determined by the resident's length of stay to determine the appropriateness of each therapy. Included in this review are nursing notes, telephone orders, lab results, behavior sheets, treatment sheets, and medication records to ensure appropriate clinical outcome. The consultant will evaluate the regulatory levels of dosing, interactions, notification of allergies, if drug monitoring is in place, that physician orders are signed timely, and PRN results are documented and usage identified. OBRA compliance for side effects are completed by the Consultant Pharmacist in conjunction with their monthly reviews, saving nurses secretarial time. The Consultant's goal is to reach the lowest maintenance dose possible and to
reduce the number of pills passed. The Consultant monitors the number of routine
medications and PRN medications per resident as compared to national averages.
Reports of all findings are forwarded to the Director of Nurses, the charge nurse on
the unit and the Attending Physician for follow up. Any recommendations not followed
up within 60 days is then noted to the Director of Nurses, Medical Director, and the Administrator.
Each month the pharmacy consultant will physically inspect a nursing station
to ensure compliance with all state regulations and appropriate medical practices.
Nursing documentation is reviewed for completeness including narcotic documentation,
medication administration sheets, and psychoactive behavior monitoring sheets. Each
month one medication pass will be completed on varying shifts.
The consultant pharmacist presents a psychotherapeutic medication usage report quarterly. This report compares the facility's psychotherapeutic drug usage statistics to the national average. This tool is used to ensure that the facility abides within the established national guidelines for psychotherapeutic drug usage. In addition to the consultant pharmacist a member of the management team from
Stat-Care Pharmacy will attend Quality Assurance Committee meetings as deemed appropriate
to ensure the quality of our services and enhance communication regarding upcoming trends and/or
regulations. This active participation allows us to address any area before it becomes
a concern.
The consultant will provide the facility with a monthly summary document detailing
activities performed by the consultant pharmacist including dates and times.
Stat-Care will surpass state requirements by providing quarterly in-service training
in house. Subjects of in-services to be determined in conjunction with the Staff
Development Department at the facility.
The consultant pharmacist will actively contribute, monitor, and assess each resident's
indidual plan of care in conjunction with their monthly drug reviews.
